Client's Rights and Responsibilities

As a client of VideoBased Counseling, you have the Right to the following:

• Have explained to you the type of services offered, time involved, fees and billing policies, before the services are rendered;

• Be informed of the professional limits and specialties of the counselor;

• Expect confidentiality;

• Help set counseling goals and be informed of your progress toward them;

• Be informed of how to contact the practitioner in an emergency situation;

• Request copies of your records;

• Terminate the professional relationship at any time

As a client of VideoBased Counseling, you have the Responsibility to:

• Set and keep ALL appointments;

• Help plan and follow through with agreed upon goals;

• Inform counselor about other counseling arrangements
